At The Chapman House we provide each resident with a personal care plan to help meet their physical and emotional needs. We offer a range of services to best meet each individual’s unique situation.
Our Expert Care Team
Our team of registered nurses (RNs), licensed practical nurses (LPNs), certified residential medication aide (CRMA), certified nursing assistants (CNAs), and personal support specialist (PSS) provide around the clock care. We strongly encourage our residents to be as independent as possible realizing that everyone’s needs may differ. We provide individualized care assisting with bathing, dressing, and grooming.

The Chapman House Clinical Team is made up of RN’s, LPN’s, CRMA’s, CNA’s and PSS’s. Our staff not only provide clinical care, but direct care with activities of daily living and emotional support for residents and families. Staff are strongly encouraged to participate in educational opportunities such as Alzheimer’s/Dementia training, wound care, and infection prevention. The investment into our staff directly impacts the level of care provided to our residents.

Senior Health Care
Medical Coordination
We maintain open lines of communication with all resident’s primary care providers and specialists. We have relationships with multiple home health agencies for residents requiring physical therapy or occupational therapy.
Appointment Transportation
We provide transportation to and from resident’s doctor and specialist appointments.
Pharmacy Coordination
We have a contract with Bedard’s Pharmacy for resident medications, or a resident can choose a pharmacy of their choice. We also have a pharmacy consultant who reviews medications on a regular basis.
Additional Services
We assess resident’s vital signs monthly, provide blood sugar monitoring per physicians orders, administer medication and treatment, and order medications. We also have a respite program, based on availability and provide podiatry services.

FAQ’s
Here are some of the questions we get most often about clinical services at Chapman.
How will I get to the doctor's office?
We transport to doctor’s offices within a certain radius. We encourage loved ones to meet residents at the doctor’s appointment. If the appointment will be over an hour, loved ones will be asked to stay at the appointment but the Chapman staff will come back and pick the residents up. Our transportation stops at 3pm.
Can I keep my medication at my bedside?
If you pass an assessment provided by an RN, residents can self administer medications. After we obtain a doctor’s order we keep the medication on the nurse cart for safety and security. Some medications can be kept at the bedside while others must be kept on the nurse cart.
Will I get my medications on time?
Medications are passed or administered by a licensed staff member per doctor’s order at prescribed times.
How will I know when to go to meals and snacks?
Staff provides reminders and escort services as-needed to daily meals and snacks.
What are visiting hours?
Visiting is from 8 am to 7 pm. Residents can go out for rides with family and loved ones at any time.
